Only a painting of his is often in circulation which dhu has provided above. Here is a rare real photograph of Chandrashekhar Azad.

A few years back I had also come across a rare photograph of his, showing his dead body in Alfred Park where he shot himself. I am unable to find that photo now. Has anyone seen that photo? I think it was from National Archives and I saw it on indiatimes.com , but I could not find it now. If somebody finds it, please provide it here.
